Metformin is a synthetic biguanide (two coupled molecules of guanidine) that is mainly absorbed in the upper small intestine and exhibits flip–flop pharmacokinetics with limited oral bioavailability 19, 20. However, reassuring data were provided in a long-term follow-up study showing similar anthropometrics in children exposed or not to metformin in utero 18. While in utero exposure to metformin was considered safe, some studies have found higher occurrence of small for gestational age birthweight and increased risk of childhood obesity 17. As metformin crosses the placenta and circulates in the developing fetus, the long-term effects of fetal exposure to metformin have raised concerns about the potential risk to growth and development of the fetus, and later, on offspring health 16. It is important to note that metformin is increasingly being used during pregnancy for the management of gestational diabetes mellitus and in those with polycystic ovary syndrome or T2DM 15. Administered orally as an immediate-release or extended-release formulation, the usual dose of metformin (0.5–2.5 g daily) is effective for long-term glycaemic control, with notable changes in HbA 1c from baseline 1, 2. Metformin is now used daily by >200 million patients with T2DM worldwide as monotherapy or in combination with sulfonylureas or dipeptidyl peptidase 4 inhibitors 1, 2. In addition, metformin moderately reduces body weight gain, possibly through upregulation of the anorectic cytokine growth differentiation factor 15 (refs. The benefits of metformin therapy in T2DM have been well documented it has long-term safety and efficacy data, low risk of hypoglycaemia, cardiovascular benefits, mortality benefits, additive or synergistic effects in combination therapy, low cost and wide availability 1, 2, 10, 11, 12. In 1957, the French physician Jean Sterne introduced the clinical use of metformin for the treatment of adult-onset diabetes mellitus 3, 4. Interestingly, in the past few years, the use of metformin has been explored for the management of patients with T2DM and obesity who contract influenza 6 or COVID-19 (refs. In 1949, metformin (called flumamine at that time) was used for the treatment of an epidemic influenza outbreak in the Philippines and was noted to lower blood levels of glucose in some of the patients with influenza 5. Metformin was first synthesized in 1922, and the first report of it being used to lower blood levels of glucose (in rabbits) was published in 1929 (refs. In this Review, we highlight the latest advances in our understanding of the mechanisms of action of metformin and discuss potential emerging novel therapeutic uses.įor the past 60 years, metformin (1,1-dimethylbiguanide hydrochloride) has been the most commonly used glucose-lowering agent and has become the first-line medication for individuals newly diagnosed with type 2 diabetes mellitus (T2DM) in many clinical guidelines 1, 2. Based on the efficacy and safety records in T2DM, attention has been given to the repurposing of metformin as part of adjunct therapy for the treatment of cancer, age-related diseases, inflammatory diseases and COVID-19. Initial studies have shown that metformin targets hepatic mitochondria however, the identification of a novel target at low concentrations of metformin at the lysosome surface might reveal a new mechanism of action. At the molecular level, it seems that the mechanisms of action vary depending on the dose of metformin used and duration of treatment. However, increasing evidence points towards other sites of action that might also have an important role, including the gastrointestinal tract, the gut microbial communities and the tissue-resident immune cells. Early evidence highlighted the liver as the major organ involved in the effect of metformin on reducing blood levels of glucose. Surprisingly, the mechanisms underlying its therapeutic action are complex and are still not fully understood. Currently, metformin is the first-line medication to treat type 2 diabetes mellitus (T2DM) in most guidelines and is used daily by >200 million patients. "What I remember vividly was a shot of the actor re-enactment of someone standing in a kitchen, and their back/shoulders caught fire and they ripped their clothing off in a frenzy," Cordell told VICE in an email. It was the example of Kay Fletcher- who claimed that in 1996, while having a peaceful morning at home, smoke and the smell of burnt flesh began to emanate from her body-that terrified Laine Cordell, an executive assistant who spent her sick days as a tween curled up with Unsolved Mysteries marathons. It’s kind of like quicksand-as a kid, I always thought quicksand was going to be more of a problem than it actually is." I even remember there’d be times that I’d feel my forehead to make sure I wasn’t feeling hot because I was afraid I would overheat and combust. "Up until I was about 14 or 15, I thought I was going to turn to ashes out of nowhere. With fire as her biggest fear, Lopez thought of spontaneous human combustion as a particularly terrible way to go, she said. I can still see it so clearly!" she told VICE in an email. "The ashes looked as though they belonged to somebody who was laying on the bed. Naissa Lopez, a communications director who recalled watching reruns of Unsolved Mysteries at night on Lifetime when she was around 10, had similar takeaways from the Mott scene. That bedroom scene is stuck in my memory forever." The bedding is red so in my mind it looked like blood, even though there wasn’t any blood in the shot, and then the way just that part of the room is burned but everything else is untouched is creepy. "In the episode, they show the bed where someone died from fire, and the room just looks so strange that it really sticks out. Though she started watching the show around 1990 at the age of 10 or 11 and loyally watched its reruns, the spontaneous combustion segment is easily the one that frightened her the most. "When anyone mentions Unsolved Mysteries, two things come to my mind: the theme song and spontaneous combustion," Brennan told VICE in an email. Detailing the experience of Kendal Mott, for example, Stack narrated a re-enactment of a man finding his father's charred remains in bed: "Fire had reduced his father to a scattering of ashes, a few splinters of bone, and a fragment of skull." Even compared to segments about murders and supernatural events, Unsolved Mysteries' spontaneous combustion segment left a fear-inducing and indelible mark on impressionable youths in the visual form of a burned bed, a walker next to a grisly piece of a leg, and a woman caught in a cloud of smoke.įor Jessica Brennan, the co-host of the California True Crime podcast, the images associated with the Mott example have stuck with her ever since. Through a series of re-enactments and interviews with alleged witnesses of spontaneous human combustion, a skeptic, and a supporter, the episode explored examples of-and made cases for and against-the idea that a human body could light itself on fire from the inside out. "Students of the paranormal call it spontaneous human combustion, or 'SHC,' when a perfectly normal person bursts into flame without warning and without apparent cause," Stack said. In one particularly memorable episode from its ninth season, which first aired in 1997 and also covered the murder of Tupac Shakur and the hunt for home invaders in Ohio, the deep-voiced host Robert Stack introduced the idea of spontaneous human combustion to the show's vast audience. Many of us with this particular horror can thank Unsolved Mysteries, the true crime documentary show that bounced around networks from 1987 to 2010, and which has found a new home on Netflix in a series reboot that launched yesterday. It was a win for Coach Odom, but also for all those who helped shape and inform Tim Duncan’s college experience as a student-athlete: former professors, coaches, teammates, trainers, team doctors, cafeteria staff, office staff, equipment managers, athletic department, Screamin’ Demons, custodians, and academic advisors. This night couldn’t get any better for a coach I would think. Coach did a great job and assured Spurs Nation that there is another community that loves Tim just as much as they do!Ĭoach Dave Odom (WFU) 7 consecutive NCAA appearancesĬoach went on to describe his first phone call with Tim and, then another, with Greg Popovich four years later, both of which changed the trajectory of Wake Forest and Spurs basketball respectively. With three kids now under 6 and living in San Antonio, I sometimes feel farther from Wake Forest than I’d like to, but as I listened to my former coach (freshman year), Dave Odom, remind 18,000 people of how all of this began with Tim, I felt close to Winston-Salem. Intuitively, Duncan knew how to encourage the person that needed encouraging. It was Tim, “let’s go to dinner man.” Manu said they talked about everything but basketball and it was exactly what he needed. Shortly thereafter, he heard another ring and quickly realized that there was also a phone in the bathroom. Manu was devastated and after receiving several phone calls in his hotel room that night, he decided to unplug the phone next to his bed as he didn’t want to talk to anyone. At one point during the playoffs several years ago, a Ginobili end of game turnover against Sacramento led to a Spurs loss. Manu Ginobili shared one of his most impactful moments with Tim. In a video, Bruce Bowen described Tim’s unique ability to take joy in other people’s success as he recalled the genuine excitement that Duncan had when Kawhi Leonard received MVP honors during their most recent championship run. Knowing the curve that Boban needed to get through to improve, Tim quickly replaced the intern and went on to play one-on-one for 20 minutes – the only catch was that Tim (at age 40) played defense the whole time. Sunday night, I felt like I was at a rehearsal dinner enjoying the gift of story without the pressure of having to speak.Īt one point during the night, Tony Parker described a moment where Tim walked through the gym and saw an intern rebounding for Boban Marjonobic, a 7 foot rookie center from Serbia. There was no vagueness, no generalities about character – only authentic recollections of a real person. The testimonies shared and speeches made were not about coach / player speak that you often hear in a typical interview in sports. It was about celebrating the effectiveness of one leader and his relentless pursuit to be the kind of teammate that his teammates needed him to be. It was about a coach who loved his player. It was about an organization where individuals get over themselves. As it turned out, this night was about relationships that have endured over a long period of time. As I watched alongside my Dad and six-year old son, I recognized that the reveal of the #21 jersey in the rafters was really a sideshow. At that point, the former All-American and, then NBA All-Star, looked at me and said, “What’s up?” I said, “Hey man” and then decided to tie my shoes some more.įast forward 16 years later, Tim Duncan’s jersey retirement ceremony was one of the more meaningful moments that I’ve experienced in sports. As I diligently tied my shoes against the wall, Tim sat down beside me to put on his shoes - he was back in Winston-Salem for a visit and decided to workout with our team. In the summer of 2000, I showed up for one of my first pick-up games as a walk-on at Wake Forest. Reflections from #21’s Jersey Retirement Ceremony: a tribute to the ultimate teammate H&R Block’s Online Assist feature offers additional virtual support with live assistance and screen-sharing, and can also be purchased on top of any of the plans starting at $70.
